Job DescriptionThe Value-based Care Manager will consult on engagements with government health and human service programs, including primarily Medicaid and Medicare programs. Engagements include the design, implementation, compliance and review of traditional and evolving health care delivery models, particularly value-based care (VBC) arrangements focused on care delivery and population health outcomes.The Value-based Care Manager will manage various consulting engagements including planning, communication, quality controls, client relationships, resource needs, training, monitoring, deliverable preparation and project evaluation.
