At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.The Float Nurse acts as a fill in for nurse coverage for all Longleaf offices, responsible for the overall management of all nursing care for assigned patients.ResponsibilitiesUse health assessment data to support Plan of CareTriage calls and assist patient/primary caregiver with problem resolution. Counsel the patient and family in meeting nursing and related needsMake visits for symptom management and support as neededModify patient treatment plans as indicated by patients' responses and conditionsMaintain current, accurate, detailed reports in accordance with policyMay monitor, record and report symptoms and changes in patients' condition to the IDGProvide health care instructions to the patient as appropriate per assessment and planObtain and receive physician orders as requiredPronounce patientsOther duties as assignedRequirementsCurrent license to practice as a registered professional nurse in Georgia.Current and continued CPR certification.At least one or two years of recent home care experience; hospice experience preferred.Good verbal and written communication and problem-solving skills.Responsible valid driver's license.
