Requirements- Current state license as a Registered Nurse - Required
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- CHPN certification - Preferred
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelor of Science in Nursing - Preferred
Responsibilities- Administer skilled nursing care for clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patients/families, and referring agencies.
- Assess patients to identify physical, psychosocial, and environmental needs.
- Implement and evaluate the Plan of Care to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety.
- Supervise clinical staff to ensure quality service delivery.
- Perform on-call responsibilities as assigned.
- Document all patient interactions and care accurately.
- Provide effective communication as a liaison between patients/families and healthcare professionals.
