By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.Home Health Occupational TherapistAssess and screen patients' daily living and work‑related skills; develop therapeutic retraining programs with measurable objectives.Administer and interpret diagnostic tests of function to assist the physician in evaluating a patient's level of function.Consult with the physician and other health‑care team members; participate in development and revision of the Plan of Care.Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range of motion, and function or to teach compensation measures.Review and expand retraining programs consistently with each patient's behavioral goals and the rehabilitation program guidelines.Consult with vocational team members to develop and implement vocationally oriented plans based on patients' needs and capabilities.Report patient status and progress to the physician, rehabilitation staff, clinical manager, patient, and family members.Participate in care coordination and discharge planning activities; act as a resource to other health‑care personnel.Design community reintegration activities to assist the client in physical reconditioning and psychological adjustment; coordinate the plan with the interdisciplinary team.Recommend and design special adaptive equipment to improve residence or working environments and participation in the rehabilitation program.Train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills.Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.Accurately document patients' care observations, interventions, and evaluations; submit treatment plans and discharge summaries to the supervisor and care management staff.Use Your Skills to Make an ImpactRequired QualificationsCurrent and unrestricted OT licensureCurrent CPR certificationValid driver's license, auto insurance, and reliable transportationPreferred QualificationsMinimum six months of occupational therapy experienceHome Health experiencePay Range$54.00–$76.00 per visit/unit$85,400–$117,500 per year base payAdditional InformationTB Statement: This role is patient‑facing and is part of Humana's Tuberculosis screening program.
