OverviewLung Transplant Advanced Practice Provider (NP/PA) - Piedmont AtlantaThe Lung Transplant Advanced Practice Provider (APP) is responsible for the comprehensive management of patients undergoing lung transplantation across the continuum of care, including pre‑transplant evaluation, inpatient hospitalization, peri‑operative management, and long‑term outpatient follow‑up.The APP functions as an integral member of a multidisciplinary transplant team that may include transplant pulmonologists, cardiothoracic surgeons, pharmacists, social workers, transplant coordinators, respiratory therapists, and nursing staff.ResponsibilitiesEssential Duties and ResponsibilitiesClinical Care:Perform comprehensive patient assessments, including histories and physical examinationsManage pre‑ and post‑lung transplant patients in both inpatient and outpatient settingsConduct daily rounds on transplant patientsEvaluate transplant candidacy and assist in pre‑transplant workupsMonitor immunosuppressive therapy and manage complicationsOrder, interpret, and evaluate laboratory studies, imaging, pulmonary function tests, and other diagnostic proceduresDevelop differential diagnoses and treatment plansPrescribe medications and therapeutic interventions in accordance with state regulations and institutional protocolsAssist with discharge planning and continuity of careCare Coordination:Collaborate with attending physicians and multidisciplinary transplant team membersCoordinate care transitions between ICU, step‑down, clinic, and home settingsParticipate in transplant selection committee meetings and multidisciplinary conferencesFacilitate communication with referring physicians and outside hospitalsPatient & Family Education:Educate patients and caregivers regarding:Lung transplant proceduresMedication regimensInfection preventionLifestyle modificationsPost‑transplant expectationsProvide ongoing counseling regarding adherence and chronic disease managementDocumentation & Compliance:Maintain timely and accurate electronic medical recordsComplete transplant evaluation summaries and discharge documentationEnsure compliance with UNOS, CMS, and institutional transplant regulationsQuality & Research:Participate in quality improvement initiativesSupport transplant program accreditation activitiesParticipate in clinical research, education, and protocol development when applicableWork Schedule:Full‑timeMix of inpatient and outpatient responsibilitiesRotating weekends and holidays requiredCall coverage required as program structure expandsQualificationsRequired Qualifications:Master's or Doctoral degree from accredited NP or PA programCurrent state licensure as:Nurse Practitioner (NP), orPhysician Assistant (PA‑C)DEA registration eligibilityBLS and ACLS certificationAbility to work in high‑acuity clinical environmentsPreferred Qualifications:Experience in:Pulmonary medicineCritical careCardiothoracic surgerySolid organ transplantationAcute Care NP certification preferred for inpatient rolesPrior transplant experience strongly preferredBusiness Unit : Company NamePiedmont Heart Institute Physicians#J-18808-Ljbffr
