Atlanta Pro Health case management has an immediate need for a Case Manager.We are looking for a qualified Case Manager to provide person-centered case management to our Medicaid members and act as a key team member of our agency. This position requires the ability to communicate efficiently, an eye for detail, experience with conducting in-person and telephonic assessments, and a problem-solving attitude. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client's health and human services needed.
