Primary ResponsibilitiesMake outbound calls and take inbound calls to assess members' current health statusIdentify gaps or barriers in treatment plansProvide patient education to assist with self-managementInteract with Medical Directors on challenging casesCoordinate care for membersEducate members on disease processesEncourage members to make healthy lifestyle changesDocument and track findingsMake post-discharge calls to ensure the member receives the necessary services and resourcesRequired QualificationsActive, unrestricted RN license in the State of GeorgiaWillingness to obtain multiple state and/or compact licensure within 60 days of hire3+ years of RN experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Case Manager for an insurance companyComputer proficiency, to include solid data entry skills and the ability to navigate a Windows environment (Word, Outlook, Excel, and Internet)Ability to travel to the Sandy Springs (Atlanta), GA office based on business needsDesignated quiet work space and access to install secure high speed internet via cable/DSL in homePreferred QualificationsBSNCertified Case Manager (CCM)Medical / Surgical, Home Health, Diabetes, Cardiac, or Emergency Room experienceExperience with acute chronic disease management for a variety of age groupsTelephonic case or disease management experienceExperience with / exposure to discharge planningA background in managed careSchedule & LocationThis is a hybrid position with onsite training for three weeks, Monday‑Friday 9am‑6pm EST. Post‑training schedule is five days per week, 11:30am‑9pm EST. Employees must be willing to attend on‑site meetings at the Sandy Springs (Atlanta), GA office as needed.
